Once you have agreed to our general terms and conditions, we will require your full medical records in English, including all x-rays, scans, recent blood work and pathology results applicable to your case.
All your medical information is needed to thoroughly assess your case. Failure to do so may result in incorrect diagnosis or suggested treatment.
Your medical records will be assessed by the attending medical team of Erasmus Medical Centre.
